General form.

As to the child [or "children"] of the parties, referred to above, it is agreed as follows:

1. Wife is to have the sole custody and control of child [or "children"] without any interference on part of husband.

2. Husband shall pay to wife $_____ each and every month [or as otherwise agreed on], beginning on _________, 19__, to be applied by wife to the support, maintenance and education of child [or "children"] until he [ or "they"] reach the age of 21.

3. Wife agrees to educate and maintain child [or "children"] in the style and manner in which he [or "she" or "they"] is now living and in which he [or "she" or "they"] has been accustomed to live.

4. Wife agrees to not remove or allow to be removed child [or "children"] more than _________ miles from the city of _________, without consent of husband.

5. After child [or "children"] attains the age of 14 years [or age agreed on] he [or "they"] shall be at liberty to reside with either of the parties at his [or "their"] election.

6. Husband shall have the right to visit child [or "children"] once a week [or as agreed] if he so desires, at such reasonable times and places as wife may designate.
